HEALTH FORM
You are required to fill out a health form. You are responsible to keep me informed of all health conditions and medications.

LATE ARRIVALS, CANCELLATIONS, NO SHOW

Late arrivals will receive an abbreviated session and will be charged for the full session that was scheduled.

24 hours cancellation notice is required. You will be charged a $25 fee if it is less than 24 hours notice.

No shows equals no more massages and a $25 fee will be charged to you.

CANCELLED OR RETURNED CHECKS

You will be responsible for all fees incurred from a cancelled or returned check.
